- 1. Acute hepatitis caused by a natural lipid-lowering product: when "alternative" medicine is no "alternative" at all.
BACKGROUND/AIMS:
The general public's growing mistrust of the pharmaceutical industry and its perception of the lack of adverse effects of "natural" therapy have lead to the increasing use of "alternative drugs" for hypercholesterolemia.
METHODS:
A sixty-three year old woman presented with severe hypertransaminasemia that had developed progressively over a few weeks. For six months she had been taking Equisterol, an over-the-counter lipid-lowering product containing guggulsterol and red yeast rice extract. The product had been prescribed for hypercholesterolemia because the patient had developed hepatotoxicity while on lovastatin.
RESULTS:
Liver biopsy revealed severe lobular necroinflammatory changes with an eosinophilic infiltrate. The episode was regarded as an adverse drug reaction after exclusion of other possible causes of acute liver disease and the prompt normalization of liver function tests after Equisterol had been discontinued. Red yeast rice extract's cholesterol-lowering properties are largely due to fungal metabolites known as monacolins, one of which--monacolin K--is identical to lovastatin.
CONCLUSIONS:
The choice of an alternative medicine approach in this case subjected the patient to "re-challenge" with the official medicine agent that had previously caused mild hepatotoxicity. Physicians should keep in mind that "alternative" medicine is not always the safest alternative and sometimes it is not even "alternative."...(more)

- 2. Nutraceuticals for treatment of high blood pressure values in patients with metabolic syndrome.
Aim: To assess whether the association of a dietary supplement with a correct diet can decrease the incidence of metabolic syndrome. In particular, we studied the effect of a combination of Ortosiphon staminensis, which in rats exerts a diuretic effect, with policosanol, red yeast rice extract, berberine, folic acid and coenzyme Q(10) on the determinants of metabolic syndrome diagnosis. Methods: The analysed sample consisted of 21 males and 9 females, who were comparable in age, in order to obtain an adequate comparison between groups with similar demographic characteristics. Thirty patients with metabolic syndrome were divided into two study arms. Both groups received the usual diet recommended by the doctor according to their clinical conditions and placebo for 2 weeks before the beginning of the study, and then they were randomly assigned to two different drug regimens: placebo or the combination of nutraceuticals previously described, and were followed-up for 6 weeks. Results: At baseline, there were no significant differences between the study and control groups for age, sex distribution, waist measurement, body mass index, systolic and diastolic blood pressure, familiarity for cardiovascular events, smoking habit, fasting glucose and lipid profile.At the end of the follow-up, only in the study group was there a significant reduction in systolic and diastolic blood pressure (-19.6±9.7 vs -3.6±8.1 mmHg and -13.6±5.5 vs -2.3±5.3 mmHg; all p < 0.0001) associated with amarked decrease in low-density lipoprotein cholesterol and triglyceride plasma levels. Consequently, 10 of 15 patients in this group no longer satisfied the criteria for metabolic syndrome, while in the control group the ratio was of 2 of 15. Conclusions: The addition of Ortosiphon staminensis to the combination of policosanol, red yeast rice extract, berberine, folic acid and coenzyme Q(10) provides an antihypertensive effect, which allows an effective control of blood pressure in patients with metabolic syndrome....(more)

- 3. Ganoderma lucidum (Reishi mushroom) for cancer treatment.
BACKGROUND:
Ganoderma lucidum is a natural medicine that is widely used and recommended by Asian physicians and naturopaths for its supporting effects on immune system. Laboratory research and a handful of preclinical trials have suggested that G. lucidum carries promising anticancer and immunomodulatory properties. The popularity of taking G. lucidum as an alternative medicine has been increasing in cancer patients. However, there is no systematic review that has been conducted to evaluate the actual benefits of G. lucidum in cancer treatment.
OBJECTIVES:
To evaluate the clinical effects of G. lucidum on long-term survival, tumour response, host immune functions and quality of life in cancer patients, as well as adverse events associated with its use.
SEARCH METHODS:
The authors ran an extensive set of databases including the Cochrane Central Register of Controlled Trials (CENTRAL), MEDLINE, EMBASE, NIH, AMED, CBM, CNKI, CMCC and VIP Information/Chinese Scientific Journals Database was searched for randomised controlled trials (RCTs) in October 2011. Other strategies used were scanning the references of articles retrieved, handsearching of the International Journal of Medicinal Mushrooms and contact with herbal medicine experts and manufacturers of G. lucidum.
SELECTION CRITERIA:
To be eligible for being included in this review, studies had to be RCTs comparing the efficacy of G. lucidum medications to active or placebo control in patients with cancer that had been diagnosed by pathology. All types and stages of cancer were eligible for inclusion. Trials were not restricted on the basis of language.
DATA COLLECTION AND ANALYSIS:
Five RCTs met the inclusion criteria and were included in this review. Two independent review authors were assigned to assess the methodological quality of individual trials. Common primary outcomes were tumour response evaluated according to the World Health Organization (WHO) criteria, immune function parameters such as natural killer (NK)-cell activity and T-lymphocyte co-receptor subsets, and quality of life measured by the Karnofsky scale score. No trial had recorded long-term survival rates. Associated adverse events were reported in one study. A meta-analysis was performed to pool available data from the primary trials. Results were gauged using relative risks (RR) and standard mean differences (SMD) for dichotomous and continuous data respectively, with a 95% confidence interval (CI).
MAIN RESULTS:
The methodological quality of primary studies was generally unsatisfying and the results were reported inadequately in many aspects. Additional information was not available from primary trialists. The meta-analysis results showed that patients who had been given G. lucidum alongside with chemo/radiotherapy were more likely to respond positively compared to chemo/radiotherapy alone (RR 1.50; 95% CI 0.90 to 2.51, P = 0.02). G. lucidum treatment alone did not demonstrate the same regression rate as that seen in combined therapy. The results for host immune function indicators suggested that G. lucidum simultaneously increases the percentage of CD3, CD4 and CD8 by 3.91% (95% CI 1.92% to 5.90%, P < 0.01), 3.05% (95% CI 1.00% to 5.11%, P < 0.01) and 2.02% (95% CI 0.21% to 3.84%, P = 0.03), respectively. In addition, leukocyte, NK-cell activity and CD4/CD8 ratio were marginally elevated. Four studies showed that patients in the G. lucidum group had relatively improved quality of life in comparison to controls. One study recorded minimal side effects, including nausea and insomnia. No significant haematological or hepatological toxicity was reported.
AUTHORS' CONCLUSIONS:

- 4. Isolation and identification of C-19 fatty acids with anti-tumor activity from the spores of Ganoderma lucidum (reishi mushroom).
We previously showed that ethanolic extracts of spores of Ganoderma lucidum inhibit tumor cell proliferation and induce apoptosis of HL-60 cells. The active constituents appeared to be long-chain fatty acids, particularly carbon-19 (C-19) fatty acids which have not been reported in spores of Ganoderma lucidum. In the present study, two of these C-19 fatty acids which are key compounds in the activities, were identified as their 2-naphthyl ester derivatives after esterification of a mixture of fatty acids obtained from the spores. The active compounds were determines as nonadecanoic acid and cis-9-nonadecenoic acid. The location of the double bond of cis-9-nonadecenoic acid was demonstrated by GC-MS analysis, based on the fragmentation pattern of the adduct prepared from the fatty acid and dimethyl disulfide.

- 5. Teratogenic and toxic effects of Lingzhi or Reishi medicinal mushroom, Ganoderma lucidum (W.Curt.:Fr.) P. Karst. (higher basidiomycetes), on zebrafish embryo as model.
This paper highlights the teratogenic and toxic effects of Ganoderma lucidum (Lingzhi or Reishi mushroom) extract on zebrafish embryos. Hatchability, malformations, and lethality rate of zebrafish embryos were assessed to provide valuable information regarding the potential teratogenic activity of G. lucidum. Hatching was completed 48 h post treatment application (hpta) at 1% or lower concentrations of extract and embryo water. The hatching rate of embryos treated with 5% or higher concentrations was significantly lower (p> 0.05) than the control. Tail malformation was the most marked morphological abnormality in embryos at 72 hpta, which was obviously caused by 1% extract (55.56% tail malformation) and was observed in all embryos exposed to 5% of extract. Growth retardation was evident in embryos exposed to 5%, 10%, and 20%. However, lethal effect of extract of G. lucidum was dependent on dose and time of exposure. Mortality rates of embryos treated with 5% (44.44%) or higher concentrations of the extract was significantly higher (p > 0.05) than that of the control embryos at 72 hpta. Beta-Glucan of medicinal Lingzhi or Reishi mushroom, Ganoderma lucidum (BGG), possesses immunostimulatory and anti-tumor activities. Innate immune cells are activated by the binding of beta-glucan to the dectin-1 receptor. The present study investigated the immunostimulating activities of BGG, including binding to dectin-1, secretion of cytokines and reactive oxygen species, and induction of Toll-like receptors (TLRs) in RAW264.7 mouse macrophages. Reverse transcription-polymerase chain reaction and flow cytometry were used for the cytokine and TLR analyses. A mouse inflammation antibody array was used for protein-level cytokine analysis. BGG bound to dectin-1 and induced RAW264.7 cell secretion of several cytokines, including granulocyte colony-stimulating factor, interleukin (IL)-6, regulated upon activation normal T cell expressed and secreted (RANTES), tissue inhibitor of metalloproteinase-1, and tumor necrosis factor-alpha. The secretion of these cytokines was further increased by the addition of lipopolysaccharide (LPS). BGG also induced both nitric oxide and inducible nitric oxide synthase (iNOS). Treatment with an inhibitor of nuclear factor-kappa B (NF-kappaB) reduced the induction of IL-1, IL-6, and iNOS in a concentration-dependent manner. Expressions of TLR2, TLR4, and TLR6 were increased by BGG treatment, and addition of LPS induced further induction of TLR4 and TLR6. Our result indicates that BGG induces macrophage secretion of inflammatory cytokines, which can be potentiated by the presence of LPS, likely by binding to dectin-1 and TLR-2/6 receptors, which activate NF-kappaB and prompt the secretion of cytokines....(more)

- 7. In vitro effects of plant and mushroom extracts on immunological function of chicken lymphocytes and macrophages.
1. The present study was conducted to examine the effects of organic extracts from milk thistle (Silybum marianum), turmeric (Curcuma longa), reishi mushroom (Ganoderma lucidum), and shiitake mushroom (Lentinus edodes) on innate immunity and tumor cell viability. 2. Innate immunity was measured by lymphocyte proliferation and nitric oxide production by macrophages, and the inhibitory effect on tumor cell growth was assessed using a non-radioactive assay. For measuring the cytokine levels in the HD11 macrophages which were treated with extracts of turmeric or shiitake mushroom, the levels of mRNAs for interferon-alpha (IFN- alpha), interleukin-1beta (IL-1beta), IL-6, IL-12, IL-15, IL-18, and tumor necrosis factor superfamily 15 (TNFSF15) were quantified by real time RT-PCR. 3. In vitro culture of chicken spleen lymphocytes with extracts of milk thistle, turmeric, and shiitake and reishi mushrooms induced significantly higher cell proliferation compared with the untreated control cells. Stimulation of macrophages with extracts of milk thistle and shiitake and reishi mushrooms, but not turmeric, resulted in robust nitric oxide production to levels that were similar with those induced by recombinant chicken interferon-gamma. All extracts uniformly inhibited the growth of chicken tumor cells in vitro at the concentration of 6.3 through 100 microg/ml. Finally, the levels of mRNAs encoding IL-1beta, IL-6, IL-12, IL-18, and TNFSF15 were enhanced in macrophages that were treated with extracts of turmeric or shiitake mushroom compared with the untreated control. 4. These results document the immunologically-based enhancement of innate immunity in chickens by extracts of plants and mushrooms with known medicinal properties in vitro. Oxidative stress and inflammation have been implicated in cerebral ischemia/reperfusion injury and complication of diabetes. The present study was designed to evaluate whether resveratrol has cerebroprotective action through antioxidant and anti-inflammatory actions in diabetic rats. Bilateral common carotid artery occlusion (30 min) and reperfusion (4 h) was employed to induce cerebral infarction in diabetic Wistar rats. Diabetes was induced by streptozocine (50 mg/kg) intraperitoneally at once. Diabetic animals were divided into groups as: normal, sham, ischemia-reperfusion, and resveratrol-treated (5, 10, 20, and 30 mg/kg). These were used for estimation of cerebral infarction. Furthermore, 20 mg/kg dose was selected for estimation of oxidative stress markers (malondialdehyde, superoxide dismutase, and catalase). Inflammatory markers like TNF-α, IL-6, IL-10, and myeloperoxidase were estimated and histological characters were studied. Resveratrol produced dose-dependent reduction in percent cerebral infarction. With resveratrol of 20 mg/kg dose, levels of oxidative stress markers and inflammatory markers like malondialdehyde, TNF-α, IL-6, and myeloperoxidase were reduced and there was a significant increase in the levels of antioxidant and anti-inflammatory markers like catalase, superoxide dismutase, and IL-10. - 10. RESOLUTION OF THE DIRECT INTERACTION AND INHIBITION OF THE HUMAN GLUT1 HEXOSE TRANSPORTER BY RESVERATROL FROM ITS EFFECT ON GLUCOSE ACCUMULATION.
Resveratrol acts as a chemopreventive agent for cancer and as a potential anti-obesity and anti-diabetic compound, by leading to reduced body fat and improved glucose homeostasis. The exact mechanisms involved in improving hyperglycemic state are not known, but most of glucose uptake into mammalian cells is facilitated by the GLUT hexose transporters. Resveratrol is structurally similar to isoflavones such as genistein, which inhibit the glucose uptake facilitated by the GLUT1 hexose transporter. Here we examined the direct effects of resveratrol on glucose uptake and accumulation in HL-60 and U937 leukemic cell lines, which express mainly GLUT1, under conditions that discriminate transport from the intracellular substrate phosphorylation/ accumulation. Resveratrol blocks GLUT1-mediated hexose uptake and thereby affects substrate accumulation on these cells. Consequently, we characterized the mechanism involved in inhibition of glucose uptake in human red cells. Resveratrol inhibits glucose exit in human red cells, and the displacement of previously bound cytochalasin B revealed the direct interaction of resveratrol with GLUT1. Resveratrol behaves as a competitive blocker of glucose uptake under zero-trans exit and exchange kinetic assays, but it becomes a mixed noncompetitive blocker when zero-trans entry transport was assayed, suggesting that the binding site for resveratrol lies on the endofacial face of the transporter. We propose that resveratrol interacts directly with the human GLUT1 hexose transporter by binding to an endofacial site and that this interaction inhibits the transport of hexoses across the plasma membrane. - 11. Simultaneous Determination of Dihydromyricetin and Resveratrol in Ampelopsis sinica (Miq.) W.T. Wang by High-Performance Liquid Chromatography Coupled with a Diode Array Detection Method.
Yeputaoteng is the dried ground part of Ampelopsis sinica (Miq.) W.T. Wang, which is widely used in traditional Chinese medicine for preventing and treating tumors, chronic nephritis, hepatitis, rubella, traumatic bleeding, stomach heat and vomiting. A simple and reliable method using high-performance liquid chromatography with diode array detection (HPLC-DAD) was developed for the simultaneous determination of dihydromyricetin and resveratrol in Yeputaoteng. The chromatographic analysis was performed on a Dikma C18 column (250 × 4.6 mm, 5 µm) at 30°C with a gradient elution of acetonitrile and 0.1% phosphoric acid at a flow rate of 1 mL/min, and used ultraviolet detection at 292 and 306 nm. The established method was validated in terms of linearity, precision, reproducibility, stability and recovery. The calibration curves showed good linear regression (R<sup>2</sup> > 0.9994). Limits of detection and quantification fell in the ranges of 1.47-2.48 and 2.93-4.97 µg/mL, respectively. The mean recovery of dihydromyricetin and resveratrol was 104.1% [relative standard deviation (RSD): 2.94%] and 100.8% (RSD: 2.80%), respectively. - 12. Resveratrol diminishes platelet aggregation and increases susceptibility of K562 tumor cells to natural killer cells.
Platelet aggregation around migrating cancer cells protects them against the activity of natural killer cells (NKCs). The inability of immune system to response results in the progression of malignant diseases. This study was designed to evaluate the effects of resveratrol (3, 4', 5-trihydroxystilbene) on platelet aggregation and NKCs activity. Experiments were designed to evaluate the platelet aggregation, production of thromboxane B2 (TXB2), estimation of expression of the platelet receptor GpIIb/IIIa (major biological markers for platelet aggregation) and functional activity of the NKCs against the K562 cancer cell line after incubation with various concentrations of reveratrol. Resveratrol at a concentration of 3 x 10(-3)M completely inhibited platelet aggregation (p < 0.05), decreased TXB2 levels (p < 0.05) and inhibited the expression of receptor GpIIb/IIIa in non-stimulated platelets (p < 0.05). At the same concentration, it increased the NKCs cytotoxic activity at an average rate of 319 +/- 34, 450 +/- 34 and 62 +/- 2.4% (p < 0.05) in the NKC/targets cells ratios of 12.5:1, 25:1 and 50:1, respectively. Thus, resveratrol not only completely inhibited platelet aggregation and reduced TXB2 levels and expression of receptor GpIIb/IIIa, but also increased the cytotoxic activity of NKCs in vitro and thus increased the susceptibility of tumor cells to NKCs. Thus, resveratrol can be used as an additional supplement to modulate the immune system and to inhibit platelet aggregation in thromboembolic episodes. - 13. Effect of Resveratrol on Cultured Skin Fibroblasts from Patients with Oxidative Phosphorylation Defects.
Few therapeutic options are available to patients with oxidative phosphorylation disorders. Administering pharmacological agents that are able to stimulate mitochondrial biogenesis have been put forward as a possible treatment, yet the approach remains in need of thorough testing. We investigated the effect of resveratrol in an in vitro setting. Mitochondrial enzymatic activities were tested in cultured skin fibroblasts from patients harboring a nuclear defect in either complex II or complex IV (n = 11), and in fibroblasts from healthy controls (n = 11). In the latter, preincubation with resveratrol resulted in a significant increase of citrate synthase, complex II and complex IV enzyme activity. In patients with complex II or complex IV deficiency, however, activity of the deficient complex could not be substantially augmented, and response was dependent upon the residual activity. - 14. Resveratrol-loaded lipid-core nanocapsules treatment reduces in vitro and in vivo glioma growth.
The development of novel therapeutic strategies to treat gliomas remains critical as a result of the poor prognoses, inef-. ficient therapies and recurrence associated with these tumors. In this context, biodegradable nanoparticles are emerging as efficient drug delivery systems for the treatment of difficult-to-treat diseases such as brain tumors. In the current study, we evaluated the antiglioma effect of trans-resveratrol-loaded lipid-core nanocapsules (RSV-LNC) based on in vitro (C6 glioma cell line) and in vivo (brain-implanted C6 cells) models of the disease. In vitro, RSV-LNC decreased the viability of C6 glioma cells to a higher extent than resveratrol in solution. Interestingly, RSV-LNC treatment was not cytotoxic to hippocampal organotypic cultures, a model of healthy neural cells, suggesting selectivity for cancer cells. RSV-LNC induced losses in glioma cell viability through induction of apoptotic cell death, as assessed by Annexin-FITC/PI assay, which was preceded by an early arrest in the S and G1 phases of the cell cycle. In brain-implanted C6 tumors, treatment with RSV-LNC (5 mg/kg/day, i.p.) for 10 days promoted a marked decrease in tumor size and also reduced the incidence of some malignant tumor-associated characteristics, such as intratumoral hemorrhaging, intratumoral edema and pseudopalisading, compared to resveratrol in solution. Taken together, the results presented herein suggest that nanoencapsulation of resveratrol improves its antiglioma activity, thus providing a provocative foundation for testing the clinical usefulness of nanoformulations of this natural compound as a new chemotherapeutic strategy for the treatment of gliomas....(more)

- 15. Development of a reagentless electrochemiluminescent electrode for flow injection analysis using copolymerised luminol/aniline on nano-TiO2 functionalised indium-tin oxide glass.
In this study, a nano-structured copolymer of luminol/aniline (PLA) was deposited onto nano-TiO2-functionalised indium tin oxide (ITO)-coated glass by electrochemical polymerisation using cyclic voltammetry (CV). The resulting reagentless electrochemiluminescent (ECL) electrode (ECLode) can be used for flow injection analysis (FIA). The properties of the ECLode were characterised by CV, electrochemical impedance spectroscopy (EIS) and scanning electron microscopy (SEM). The ECLode has high background ECL emission as well as excellent stability and reproducibility, and yielding sensitive response towards target analytes. The ECL emissions of the ECLode were 50 times higher than PLA/ITO, and 500 times higher than polyluminol (PL)/ITO. The ECLode showed sensitive responses to reactive oxygen species (ROSs), permitting its application for determination of antioxidants by quenching. Under optimised conditions, an absolute detection limit of 69.9pg was obtained for resveratrol, comparable to the highest levels of sensitivity achieved by other methods. Thus, the gross antioxidant content of red wine was determined, with satisfactory recoveries between 87.6% and 108.3%. - 16. Does resveratrol improve insulin signaling in chronically ischemic myocardium?
BACKGROUND:
Resveratrol is a naturally occurring polyphenol believed to be cardioprotective. We previously demonstrated that resveratrol improves insulin signaling and glucose metabolism in liver and skeletal muscle of swine with metabolic syndrome. Although resveratrol has metabolic benefits in peripheral tissues, its effect on insulin signaling in ischemic myocardium (IM) is unclear. Therefore, we developed a clinically relevant swine model of metabolic syndrome and chronic myocardial ischemia to investigate the effects of resveratrol on insulin signaling in cardiac tissue.
MATERIALS AND METHODS:
Thirteen male Yorkshire swine were fed a high-cholesterol diet for 4 wk then underwent surgical placement of an ameroid constrictor to their circumflex artery to induce chronic myocardial ischemia. The high-cholesterol control group was given no drug (n = 7). The experimental group was provided the same diet and received supplemental resveratrol (100 mg/kg/d) (n = 6). Tissue was harvested 7 wk after ameroid placement for western blot and histological analyses.
RESULTS:
In IM, there was no significant difference between the two groups in the insulin signaling markers studied. In nonischemic myocardium, there was a significant decrease in phosphorylated AMP-activated protein kinase α (P = 0.021) in the group treated with resveratrol; otherwise, there were no significant differences between the groups. Immunostaining for glucose transporter 4 and Periodic acid-Schiff staining for myocardial glycogen stores was similar between the groups.
CONCLUSIONS:
Resveratrol has complex effects on glucose metabolism. Although prior studies demonstrated that resveratrol supplementation improves insulin sensitivity in peripheral tissues, in chronically IM, there are no significant alterations.

- 18. Resveratrol vs. calorie restriction: Data from rodents to humans.
Calorie restriction extends lifespan and confers metabolic benefits similar to the effect of lifestyle interventions. Poor compliance to long-term dietary restriction, however, hinders the success of this approach. Evidence is now persuasive for a role of resveratrol supplementation (a polyphenol in red grapes) as potential alternative to calorie restriction. This review summarizes the latest literature on the effects and the molecular mechanisms by which calorie restriction and resveratrol confer health benefits. The activation of SIRT1 and the associated improvement in energy utilization and insulin sensitivity closely resemble the benefits of calorie restriction. Current data largely support resveratrol as a potential calorie restriction mimetic to improve metabolic and probably functional health. - 19. Argirein alleviates corpus cavernosum dysfunction by suppressing pro-inflammatory factors p66Shc and ER stress chaperone Bip in diabetic rats.
OBJECTIVES:
The aim was to investigate whether argirein, which releases rhein and L-arginine after medication, could improve erectile dysfunction (ED) in diabetic rats through normalising the abnormalities of nitric oxide synthase (NOS), p66Shc and immunoglobulin heavy-chain binding protein (Bip), in the corpus cavernosum (CC).
METHODS:
SD rats were randomly divided into six groups. Except for the control group, rats were injected with streptozotocin (STZ) (60 mg/kg, i.p.) once. During weeks 5-8 following STZ injection, except for STZ-injected untreated rats, others were treated with aminoguanidine (AMG; 100 mg/kg/day, i.g.), or argirein at three doses (50, 100 and 200 mg/kg/day, i.g.). The vascular activity and biomarkers of the cavernosum were examined.
KEY FINDINGS:
Constrictive and dilative activity was abnormal in the CC, associated with decreased nitric oxide (NO) in serum in the diabetic (DM) group. Increased expression of p66Shc, Bip and inducible nitric oxide synthase (iNOS) and decreased endothelial nitric oxide synthase (eNOS) in the CC were significant in DM rats. Argirein and AMG improved these abnormities significantly.
CONCLUSIONS:
We concluded that vascular activity of the cavernosal tissue was impaired due to upregulated p66Shc and Bip in the diabetic CC. Argirein alleviates the vascular dysfunction of the CC by suppressing these upregulated pro-inflammatory proteins caused by diabetic lesions.

- 20. Studies on a rhein-producing endophytic fungus isolated from Rheum palmatum L.
Rheum palmatum L. (Chinese rhubarb) is a highly regarded medicinal plant. Its dominant active constituents are anthraquinones including emodin, aloe-emodin, rhein, etc. Rhein naturally occurs in anthraquinone (1, 3, 8-trihydroxy-6-methyl anthraquinone), which is found in R. palmatum L. and related plants such as rhubarb. It has good antitumor, anti-inflammatory, anticancer, antimicrobial and hemostatic properties. In this study, a total of 14 strains of endophytic fungi were isolated from R. palmatum L. All fungal isolates were fermented in liquid PDA medium and their extracts were preliminarily analyzed by antibacterial reactions, magnesium acetate-methanol reagent and Borntraiger's reaction, and the strain reselection was made by thin-layer chromatography (TLC), high-performance liquid chromatography (HPLC) and LC-MS to identify the fermentation products of the selected strains and confirmed through a comparison with authentic standards. Extract from one strain, R13, showed positive reactions with both reagents. The strain R13 had a component with the same TLC (Rf) value and HPLC, LC-MS retention time as authentic rhein standards. - 21. Anthraquinone-2-sulfonic acid (AQ2S) is a novel neurotherapeutic agent.
Anthraquinone derivatives such as emodin have recently been shown to protect in models of beta amyloid β (Aβ) and tau aggregation-induced cell death. The mechanisms of action possibly involve preconditioning effects, anti-aggregation properties, and/or enhancing the phosphatidylinositol-3-kinase (PI3K)/AKT survival mechanism. We studied several natural (emodin, rhein, and aloin) and synthetic (AQ2S) anthraquinones, to screen for post-treatment therapeutic benefit in two models of neuronal death, namely hydrogen peroxide (H(2)O(2)) and staurosporine (STS)-induced injury. Treatment with emodin, rhein, or aloin failed to reduce H(2)O(2) injury. Moreover, consistent with emodin behaving like a mild toxin, it exacerbated oxidative injury at the highest concentration used (50 μM) in our post-treatment paradigm, and potently inhibited AKT. In contrast, AQ2S was neuroprotective. It reduced H(2)O(2) injury at 50 and 75 μM. In addition, AQ2S potently inhibited staurosporine (STS)-induced injury. The mechanisms of action involve caspase inhibition and AKT activation. However, blockade of AKT signaling with LY294002 failed to abolish AQ2S-mediated protection on the STS assay. - 22. Comparative pharmacokinetics study of three anthraquinones in rat plasma after oral administration of Radix et Rhei Rhizoma extract and Dahuang Fuzi Tang by high performance liquid chromatography-mass spectrometry.
A specific and sensitive high performance liquid chromatography-mass spectrometric (HPLC-MS) method has been developed and validated for simultaneous determination of three anthraquinones of rhein, aloe-emodin and emodin in rat plasma after oral administration of Radix et Rhei Rhizoma extract and Dahuang Fuzi Tang. The analytes were separated on a Kromaisl(®) C(18) column within a total running time of 12min with a mobile phase of methanol:ammonium acetate (3mM) (75:25, v/v). The calibration curves for all the anthraquinones showed good linearity in the measured range with correlation coefficient (r) higher than 0.9978. The precision, accuracy, recovery and stability were deemed acceptable. - 23. Metabolomics Coupled with Proteomics Advancing Drug Discovery toward More Agile Development of Targeted Combination Therapies.
To enhance the therapeutic efficacy and reduce the adverse effects of traditional Chinese medicine, practitioners often prescribe combinations of plant species and/or minerals, called formulae. Unfortunately, the working mechanisms of most of these compounds are difficult to determine and thus remain unknown. In an attempt to address the benefits of formulae based on current biomedical approaches, we analyzed the components of Yinchenhao Tang, a classical formula that has been shown to be clinically effective for treating hepatic injury syndrome. The three principal components of Yinchenhao Tang are Artemisia annua L., Gardenia jasminoids Ellis, and Rheum Palmatum L., whose major active ingredients are 6,7-dimethylesculetin (D), geniposide (G), and rhein (R), respectively. To determine the mechanisms underlying the efficacy of this formula, we conducted a systematic analysis of the therapeutic effects of the DGR compound using immunohistochemistry, biochemistry, metabolomics, and proteomics. Here, we report that the DGR combination exerts a more robust therapeutic effect than any one or two of the three individual compounds by hitting multiple targets in a rat model of hepatic injury. - 24. Developmental Changes in the Composition of Five Anthraquinones from Rheum palmatum as Quantified by (1) H-NMR.
INTRODUCTION:
Rheum palmatum is an important traditional Chinese medicine featuring anthraquinones with several activities. Generally, rhein, emodin, aloe-emodin, physcion and chrysophanol are used as chemical markers for the quality control of rhubarb products.
OBJECTIVE:
To develop a simple protocol for the quantification of rhein, emodin, aloe-emodin, physcion and chrysophanol in R. palmatum collected at different developmental stages.
METHODS:
(1) H-NMR spectra were measured on samples dissolved in acetone-d(6) , quantification was carried out using the signals of H-4 of rhein (δ(H) 8.36), H-7 of emodin (δ(H) 6.68), CH(2) OH of aloe-emodin (δ(H) 4.81), OCH(3) of physcion (δ(H) 4.02) and CH(3) of chrysophanol (δ(H) 2.50), which were well separated from other signals. Quantitative analysis was based on the relative ratio of the intensity of each compound to the known amount of internal standard maleic acid.
RESULTS:
The quantitative (1) H-NMR (qHNMR) method developed showed good precision, trueness, linearity, repeatability and stability for the quantification of rhein, emodin, aloe-emodin, physcion and chrysophanol. This method was applied successfully to explore the seasonal variations of the five major anthraquinones in R. palmatum, and provided quantitative results in reasonable agreement with those obtained by the HPLC-UV method.
CONCLUSION:
Compared with the conventional HPLC-based methods, the qHNMR analysis is rapid, reference-free and convenient with less sample pre-treatment. This technique should be a feasible choice for the quality control of R. palmatum. Copyright © 2013 John Wiley & Sons, Ltd.

Abstract Context: Qi-Wei-Xiao-Yan-Tang (XYT), composed of Radix et Rhizoma Rhei, Radix Astragali, Radix Wikstroemiae Indicae, Fructus Ligustri Lucidi, Poria and Radix Glycyrrhizae, has been widely used as an anti-inflammatory drug. Objective: The present study investigated the anti-inflammatory, antibacterial effects and serum pharmacochemistry of XYT. Methods: The dimethylbenzene-induced inflammation test, the acetic acid-induced vascular permeability test and the carrageenan-induced paw edema test were used to evaluate the anti-inflammatory activity of XYT (200, 100 and 50 mg/kg); minimal inhibitory concentration (MIC). Minimal bactericidal concentration (MBC) tests were used to evaluate the antibacterial activity of XYT. Additionally, serum pharmacochemistry was performed to study the biologically active substances. Results: All the tests for anti-inflammatory effects were shown active with these test systems; the anti-inflammatory effects at doses of 100 and 200 mg/kg were significant (p < 0.05); MIC and MBC tests indicated that XYT showed a broader antimicrobial spectrum and stronger toxicity to the tested microbes. Additionally, calycosin-7-glucoside, sennoside A, aloeemodin and rhein were detected as the predominant components in rat serum which may play the key role in the anti-inflammatory activities of XYT. Conclusions: This is the first report of the pharmacological activities and serum pharmacochemistry of XYT, and the first evidence of anti-inflammatory and antimicrobial properties of the extracts of XYT. The results of our work demonstrated that XYT has significant anti-inflammatory and antibacterial properties, and calycosin-7-glucoside, sennoside A aloeemodin and rhein may be the biologically active substances of XYT. In order to study the hydrolytic characterization of an anti-inflammatory prodrug (RD-1) in vitro, a simple, specific, precise and accurate method for the simultaneous determination of prodrug and its two hydrolytic active compounds was development using reverse phase high-performance liquid chromatography (RP-HPLC). The chromatographic separation was performed on an ODS-2 C18 column (250 mm x 4.6 mm, 5.0 microm particle size) with a simple elution programme. The mobile phase was methanol-0.1%phosphoric acid solution (adjusted pH to 2.3) (80:20, v/v); wavelength of 257nm and mobile phase flow rate of 1.0 mL/min was utilized for the quantitative analysis. Aim:To investigate the protective effects of rhein lysinate (RHL), a major bioactive constituent of the rhizome of rhubarb (Rheum palmatum Linn or Rheum tanguticum Maxim), against kidney impairment in senescence-prone inbred strain 10 (SAMP10) mice.Methods:SAMP10 mice were orally administered RHL (25 or 50 mg/kg) daily until 50% of the mice died. Senescence-resistant inbred strain 1 (SAMR1) mice administered no drug were taken as control. The kidneys were harvested after animal death, and examined morphologically and with immunochemical assays. The levels of MAD, SOD and GSH-px in the kidneys were measured with a photometric method. The expression of inflammatory factors and related proteins in the kidneys was analyzed using Western blotting.Results:Treatment of SAMP10 mice with RHL had no effect on the body weight or phenotype. However, RHL significantly prolonged the median survival time of SAMP10 mice by approximately 25%, as compared to untreated SAMP10 mice. Compared SAMR1 mice, SAMP10 mice had a significantly lower level of SOD in the kidneys, but had no significant difference in the MDA or GSH-px levels. Treatment of SAMP10 mice with RHL significantly reduced the MAD level, and increased the SOD and GSH-px levels in the kidneys. Glomerulonephritis was observed in SAMP10 mice but not in SAMR1 mice. RHL decreased the incidence of glomerulonephritis, and significantly decreased the levels of TNF-α, IL-6, NF-κB, collagen types I and III in the kidneys.Conclusion:Accelerated senescence is associated with glomerulonephritis in SAMP10 mice, and RHL prolongs their median survival time by reducing the severity of glomerulonephritis....(more)

- 28. Simultaneous Preconcentration and Analysis of Anthraquinones Based on Ultrasound Emulsification Ionic Liquid Microextraction.
An ultrasensitive method of ultrasound emulsification ionic liquid microextraction (UEILME) coupled with high-performance liquid chromatography (HPLC) has been developed and introduced for the preconcentration and analysis of anthraquinone additives in cosmetic samples and five anthraquinone compounds (aloe-emodin, rhein, emodin, chrysophanol and physcion) in traditional Chinese medicines. Several parameters affecting the extraction efficiency were investigated and optimized, such as the type and amount of extraction solvent, sample pH, ultrasound time and temperature, centrifugation speed and time and ionic strength. The most favorable results were obtained using 60 mg of 1-hexyl-3-methylimidazolium hexafluorophosphate as extraction solvent. The anthraquinones were extracted from the aqueous solution (pH 2.0) by ultrasound at 40°C for 7 min and centrifuged at 2,500 rpm for 6 min. Under optimal conditions, acceptable linearity of the five anthraquinone compounds was obtained with correlation coefficients > 0.99. The limits of detection (LODs) and limits of quantitation (LOQs) ranged from 0.01 to 0.09 µg/L and 0.05 to 0.25 µg/L, respectively. The relative standard deviations (n = 3) were less than 9.8%. Moreover, the enrichment factors ranged from 80 to 197-fold. Compared with conventional dispersive liquid-liquid microextraction, the UEILME technique exhibited lower LODs and LOQs. The results demonstrated that the UEILME coupled with HPLC is a simple, environmentally friendly, sensitive and efficient method for the extraction, concentration and analysis of anthraquinone compounds....(more)

OBJECTIVE:
To establish a rat model of diabetes-associated cerebral ischemia due to qi and yin deficiency and blood stasis, and to investigate the effects of Radix Ginseng, Rhizoma Chuanxiong, Rhizoma Polygonati Odorati and Rhizoma Polygonati Sibirici Granule (Shenxiong Yujing Granule), which has the function of strengthening qi, nourishing yin, and activating blood, on proliferation, differentiation and survival of neural cells in rats with diabetes-associated cerebral ischemia.
METHODS:
Rats were divided into sham-operation, diabetes plus ischemia reperfusion injury model, Shenxiong Yujing Granule and Radix Ginseng and Rhizoma Chuanxiong Granule (Shenxiong Granule) groups with 20 rats in each. The 5-bromo-2'-deoxyuridine (BrdU) incorporation assay and immunohistochemical method were used to investigate the proliferation, differentiation and survival of neural cells in dentate gyrus of rats with diabetes-associated cerebral ischemia.
RESULTS:
The number of newly proliferating cells in subgranular zone of dentate gyrus was increased in the model group, but there was no significant difference compared with 7 day treatment with Shenxiong Yujing Granule. Shenxiong Yujing Granule significantly increased the survival rate and promoted the differentiation of newly proliferating neurons after 21-day treatment (P<0.01). In addition, the beneficial effect of Shenxiong Yujing Granule was considerably greater than that of the Shenxiong Granule (P<0.01).
CONCLUSION:
Shenxiong Yujing Granule can increase the survival rate and promote the differentiation of newly proliferating neurons in rats with diabetes-associated cerebral ischemia of dual deficiency of qi and yin and blood stasis obstructing the collaterals. - 30. Negative regulation of adipogenesis by kaempferol, a component of Rhizoma Polygonati falcatum in 3T3-L1 cells.
Rhizoma Polygonati falcatum (RPF) has been used as a traditional herbal medicine in Asia, because of its anti-hyperglycemic, anti-triglycemic, and anti-tumor activity. In this study, we determined the anti-adipogenic potential of RPF extract and its component kaempferol in 3T3-L1 adipocytes, and the underlying molecular mechanism(s) using microarray analysis. Adipocyte differentiation of 3T3-L1 cells was significantly impaired by RPF extract and kaempferol as monitored by Oil Red O staining and quantitative measurement of lipid accumulation. Additionally, the mRNA expression of adipogenesis genes decreased on treatment with kaempferol. The role of kaempferol at the genome-wide level was further assessed by a microarray approach. Our analysis indicated that kaempferol decreased the expression of adipogenic transcription factors (Pparγ, Cebpβ, Srebp1, Rxrβ, Lxrβ, Rorα) and genes involved in triglyceride biosynthesis (Gpd1, Agpat2, Dgat2), while increasing lipolysis-related genes, such as Tnfα, Lsr, and Cel. Finally, co-transfection assays using luciferase reporter gene and reverse transcription-polymerase chain reaction (RT-PCR) analysis using peroxisome proliferator-activated receptor-γ (PPARγ) target genes indicated that kaempferol significantly repressed rosiglitazone-induced PPARγ transcriptional activity. - 31. Effect of Rhizoma Polygonati on 12-O-tetradecanoylphorbol-acetate-induced ear edema in mice.
ETHNOPHARMACOLOGICAL RELEVANCE:
Rhizoma Polygonati is originated from the dried rhizomes of Polygonatum sibircum Red. It has long been used in traditional Chinese medicine for the treatment of inflammatory disorders.
AIM OF THE STUDY:
The present study aims to investigate the anti-inflammatory effect of aqueous extract of Rhizoma Polygonati (ERP) in a mouse model of inflammation induced by 12-O-tetradecanoylphorbol-acetate (TPA).
MATERIALS AND METHODS:
The anti-inflammatory effect was evaluated by measuring the ear thickness and activity of myeloperoxidase (MPO). The anti-inflammatory mechanism was explored by determining the protein and mRNA levels of inducible nitric oxide synthase (iNOS), cyclooxygenase-2 (COX-2), tumor necrosis factor-α (TNF-α), interleukin (IL)-1β, and IL-6.
RESULTS:
The results showed that ERP significantly decreased the ear thickness and MPO activity in mouse model of inflammation induced by TPA. In addition, ERP also remarkably inhibited the protein and mRNA levels of iNOS, COX-2, TNF-α, IL-1β, and IL-6.
CONCLUSIONS:
These results indicate that ERP has potential anti-inflammatory effect on TPA-induced inflammatory in mice, and the anti-inflammatory effect may be mediated, at least in part, by inhibiting the mRNA expression of a panel of inflammatory mediators including iNOS, COX-2, TNF-α, IL-1β, and IL-6.

- 32. [Study on the composition and pharmacological function change of rhizoma polygonati before and after processing].
OBJECTIVE:
To discuss the processing mechanism of Rhizoma Polygonati (RP) through studying the correlation between the change of composition and pharmacological function in raw and processed RP.
METHODS:
The extraction of petroleum ether, methylene dichloride, ethyl acetate and 1-butanol of the raw and processed RP were compared by HPLC. The compounds changed in processed RP in the methylene dichloride extraction were further identified with reference substances. The immune function of methylene dichloride extraction of raw and processed RP were compared.
RESULTS:
The changed compound in concentration was determined to be 5-Hydroxymethylfurfural. After processed, the concentration of 5-Hydroxymethylfurfural sharply increased. The carbon clearance index (P < 0.01) and coefficient of phagocytosis (P < 0.05) were increased remarkably by processed RP comparing to those of the normal saline and raw RP.

- 33. [Quality standard of stewed rhizoma polygonati with yellow wine].
OBJECTIVE:
To improve the quality standard of stewed Rhizoma Polygonati with yellow wine.
METHODS:
On the basis of Chinese Pharmacopoeia 2010, some items were added, which included assay of reducing sugars, water-soluble extracts, acid insoluble ash content and 5-hydroxymethylfurfural.
RESULTS:
Reducing sugars was no less than 34.0%, water-soluble extracts no less than 19.0%, acid insoluble ash content no more than 0.2%, 5-hydroxymethylfurfural no more than 0.02%.
CONCLUSION:
The quality standard of stewed Rhizoma Polygonati with yellow wine is improved on the basis of Chinese Pharmacopoeia....(more)

- 34. Diabetes is an inflammatory disease: evidence from traditional Chinese medicines.
Diabetes is usually associated with inflammation. Inflammation contributes to the development of diabetes. Traditional Chinese medicines (TCM) play an important role in lowering blood glucose and controlling inflammation. Many studies show that TCM with hypoglycaemic effects, for example Radix Astragali, Radix Rehmanniae, Radix Trichosanthis, Panax Ginseng, Fructus Schisandrae, Radix Ophiopogonis, Rhizoma Anemarrhenae, Radix Puerariae, Fructus Lycii, Poria, Rhizoma Coptidis, Rhizoma Dioscoreae, Rhizoma Polygonati, Radix Salviae Miltiorrhizae, Radix Glycyrrhizae, Semen Trigonellae, Momordica charantia, Allium sativum, Opuntia stricta, Aloe vera, Cortex Cinnamomi, Rhizoma Curcumae Longae, and so on, have nearly independent anti-inflammatory action. Antihyperglycaemic compounds, for example berberine, puerarin, quercetin, ferulic acid, astragaloside IV, curcumin, epigallocatechin gallate, resveratrol, tetrandrine, glycyrrhizin, emodin and baicalin, used in TCM also have anti-inflammatory effects. These studies suggest that TCM might exert hypoglycaemic effects that are partly mediated by the anti-inflammatory mechanisms. However, small amounts of TCM with potent anti-inflammatory action does not have any hypoglycaemic effect. This indirectly indicates that diabetes may be a low-grade inflammatory disease and potent regulation of inflammatory mediators may not be required. Studies of TCM add new evidences, which indicate that diabetes may be an inflammatory disease and slight or moderate inhibition of inflammation might be useful to prevent the development of diabetes. Through this review, we aim to develop more perspectives to indicate that diabetes may be an inflammatory disease and diverse TCM may share a common antidiabetic property: anti-inflammatory action. Further studies should focus on and validate inflammation-regulating targets of TCM that may be involved in inhibiting the development of diabetes.

- 35. [Effects of electroacupuncture plus oxygenmedicine on the expression of Bcl-2 and Bax proteins in the hippocampal CA 1 area in rats with global cerebral ischemia/reperfusion injury].
OBJECTIVE:
To observe the effect of electroacupuncture (EA) plus oxygenmedicine (OM) on the expression of Bcl-2 and Bax in the hippocampal CA 1 area in cerebral ischemia/reperfusion injury (CI/RI) rats.
METHODS:
Thirty SD rats were randomized into sham-operation, model, EA, OM, EA+OM groups (n=6 /group). CI/RI model was established by using modified Pulsinelli 4 vessel occlusion and reperfusion. EA (100 Hz, 3.5 mA) was applied to "Baihui" (GV 20) and "Zusanli" (ST 36) 30 min, once daily for 4 days. Rats of OM and EA+ OM groups were put into a box filled with oxygen and atomized herbal medicines containing Bingpian (Borneolum), Shexiang (Moschus), Huangjing (Rhizoma Polygonati), Shouwu (Radix Polygoni Multiflori), etc. for 30 min, once daily for 4 days. Bcl-2 and Bax expression of the hippocampal CA 1 area was detected by immunohistochemistry.
RESULTS:
Compared with sham group, the numbers of Bcl-2 immunoreaction (IR) and Bax IR positive cells, and the immunoactivity of Bcl-2 IR and Bax IR positive products in the hippocampal CA 1 area were increased significantly in model group (P < 0.05, P < 0.01). In comparison with model group, the number of Bcl-2 IR positive cells and Bcl-2 immunoactivity in EA, OM and EA+OM groups increased considerably (P < 0.01), while Bax IR positive cell numbers and Bax immunoactivity in EA, OM and OM+ EA groups decreased significantly (P < 0.01). The effects of EA+ OM were significantly superior to those of EA and OM groups in upregulating Bcl-2 IR positive cell number and Bcl-2 immunoactivity and downregulating Bax IR positive cell number and Bax immunoactivity (P < 0.01). No significant differences were found between EA and OM groups in the abovemen-EA and OM and EA+OM can effectively regulate the expression of Bcl-2 and Bax in tioned indexes (P > 0.05).
CONCLUSION:
EA and OM and EA + OM can effectively regulate the expression of Bcl-2 and Bax in the hippocampal CA 1 area in CI/RI rats, and the effects of EA+OM are significantly superior to those of simple EA and simple OM, which may contribute to their effect in improving cerebral ischemia....(more)
